include appName=DbVis-8.0.1 as default for jTDS/Sybase JDBC connections

DbVisualizer should include a default 'appName' property if one isn't supplied for the jdbc connection. (e.g. like Aqua Data Studo or DbArtisan) This would help identify DbVis clients from other processes when looking at connections on the database. (for Sybase and jTDSat least, i'm guessing there is a similar mechanism for other Databases) Currently connections from DbVis just show 'program_name'=null by default. I realize you can configure the value for 'APPLICATIONNAME' on the the properties tab, but most people don't bother, and it would be nice to have 'DbVis-Version' as a sensible default. It also looks like it needs to be done for each connection. (I didn't find a place to change it globally)
